<p>Occupational Therapists aim to support posture in ways that enhance comfort, function, and participation. Whether correcting or accommodating posture, the goal is always to improve quality of life across all environments, including beds, wheelchairs, armchairs, and vehicles.</p>
<h4><strong>Postural Alignment vs. Postural Perfection</strong></h4>
<p>Good alignment is desirable, but postural perfection isn’t always realistic. Each client’s tolerance, physical capacity, and lived experience must be considered. A comprehensive postural assessment—conducted in various positions such as lying, sitting, and floor-based—is essential to determine whether posture can be corrected or must be accommodated.</p>
<h4><strong>Tailored Equipment and Gradual Implementation</strong></h4>
<p>Once needs are identified, selecting the right postural support equipment is critical. With many similar products on the market, trialing is essential to ensure the best fit. For clients with complex postural needs, a <strong>graded introduction</strong> may be necessary to support tolerance and acceptance.</p>

Awareness of posture-related risks, such as pressure injuries, respiratory compromise, and reduced independence, is essential for proactive care.</p>

<p>Multiple systems influence posture: musculoskeletal, neurological, sensory, behavioural, and more. No single clinician can address all these areas in isolation. A collaborative approach ensures that:</p>

<p>Posture is far more than how we sit or stand, it’s the silent architect of how we move, communicate, and engage with the world. For Occupational Therapists, posture is a foundational element that underpins participation in everyday life. Yet, it’s often overlooked until it begins to interfere with function.</p>
<p>In many cases subtle postural deviations, like rounded shoulders or asymmetrical weight-bearing, don’t immediately impact a person’s ability to participate in daily activities. But when these issues become more pronounced, they can significantly affect independence, comfort, and overall well-being.</p>
<h3><strong>Why Posture Matters in Neurological Conditions</strong></h3>
<p>Posture is shaped by a complex interplay between the brain, spinal cord, and musculoskeletal system. In neurological conditions, this relationship is often disrupted, leading to challenges such as:</p>
<ul>
<li><strong>Motor impairments</strong> (e.g., balance issues, musculoskeletal limitations)</li>
<li><strong>Muscle tone abnormalities</strong> (e.g., weakness, paralysis, imbalances)</li>
<li><strong>Sensory processing difficulties</strong></li>
<li><strong>Cognitive and behavioural factors</strong></li>
<li><strong>Fatigue and endurance limitations</strong></li>
</ul>
<p>These factors can result in postural challenges that go beyond mobility. For example, clients with spinal cord injury (SCI) or cerebral palsy (CP) may struggle not only with movement but also with basic tasks like brushing their teeth or reaching for items, activities that are essential for independence and dignity.</p>
<h3><strong>The Ripple Effect of Poor Posture</strong></h3>
<p>Posture doesn’t just affect movement, it influences nearly every system in the body:</p>
<ul>
<li><strong>Respiratory function</strong>: Poor posture can restrict lung expansion, reducing breathing efficiency.</li>
<li><strong>Digestive health</strong>: Kyphosis or scoliosis can compress abdominal organs, slowing digestion.</li>
<li><strong>Circulation</strong>: Inadequate positioning can impair blood flow and contribute to swelling.</li>
<li><strong>Psychological well-being</strong>: Loss of independence due to postural limitations can lead to low mood and reduced motivation.</li>
</ul>
<p>This is why <strong>24-hour posture management</strong> is essential. Posture must be supported across all environments—wheelchair, armchair, bed, toilet, shower, car, kitchen—not just during therapy sessions. Every position matters.</p>
<h3><strong>The Role of OT in 24-Hour Posture Management</strong></h3>
<p>Clients with limited mobility or tone abnormalities often cannot reposition themselves independently. Over time, this can lead to postural deformities, discomfort, and serious complications such as pressure injuries or respiratory issues.</p>
<p>Occupational Therapists play a critical role in:</p>
<ul>
<li><strong>Assessing postural needs across all settings</strong></li>
<li><strong>Recommending specialist seating and sleeping systems</strong></li>
<li><strong>Educating caregivers and support teams</strong></li>
<li><strong>Balancing support with freedom of movement</strong></li>

<p>It’s important to challenge the misconception that supportive systems restrict clients. In reality, poor posture may be the reason a client appears unmotivated or fatigued, when in fact, they’re physically unable to lift their arms due to muscle imbalance and poor trunk alignment.</p>
<p>Posture is not a passive concern, it’s an active determinant of health, function, and quality of life. As Clinical Managers and industry peers, we must advocate for comprehensive posture management strategies that span the full 24-hour cycle. This means investing in education, equipment, and interdisciplinary collaboration to ensure our clients are supported in every position, every day.</p>

<p>In July, our occupational therapists and OTA attended the <a href="https://www.pmguk.co.uk/conference" target="_blank" rel="noopener">Posture &amp; Mobility Group (PMG) Conference 2025</a>‚ a leading event for professionals working in posture, seating, and mobility. The conference provided a platform for clinical learning, innovation, and collaboration.  Our team returned with renewed insight and practical strategies to enhance outcomes for individuals with complex physical needs.</p>
<p>&nbsp;</p>
<h3>Jo&#8217;s Thoughts: A Holistic Approach to Postural Care</h3>
<p>Jo, one of our senior therapists, emphasised the importance of adopting a <strong>24-hour postural care model</strong>‚ a shift that considers not only seating, but also lying and standing positions throughout the day and night.</p>
<blockquote><p>Even with extensive experience in this field, there&#8217;s always more to learn. The conference really reinforced how vital it is to look at postural care across the full 24 hours‚ not just when someone is seated. That shift in thinking helps us make more meaningful, person-centred recommendations.</p></blockquote>
<p>Jo&#8217;s learning has already informed her clinical reasoning, particularly in assessing complex presentations such as high muscle tone, scoliosis, and progressive neurological conditions. She&#8217;s also integrating <strong>pressure mapping</strong> and other advanced tools to support decision-making and prevent pressure injuries. Crucially, Jo is placing greater emphasis on <strong>collaborative care</strong>, ensuring families and carers are actively involved in assessments and fittings.</p>

<h3>Kate&#8217;s Highlights: Adaptive Design and AI Integration</h3>
<p>Kate was inspired by sessions that showcased <strong>real-world innovation</strong> and the value of iterative design:</p>

<li>A case study involving a gentleman who had lost both upper limbs demonstrated how <strong>continuous adaptation of hand controls</strong> led to a solution that was comfortable, sensitive, and accurate.</li>
<li>Another session explored <strong>AI-driven wheelchair technology</strong>, where users could steer using headrests and foot pedals. The system intelligently filtered out accidental movements, responding only to purposeful gestures.</li>

<blockquote><p>It&#8217;s amazing to see how technology is helping people gain independence‚ especially those who don&#8217;t have hand mobility. Using specialised glasses or head and foot movements to control a power chair is just brilliant.</p></blockquote>

<p>Since returning from PMG, our team has been actively applying what we&#8217;ve learned:</p>

<li><strong>Trialling new equipment</strong> with suppliers met at the conference, including adaptable seating systems and pressure-relieving cushions.</li>
<li><strong>Enhancing assessments</strong> with a more holistic, evidence-informed approach.</li>
<li><strong>Strengthening collaboration</strong> with families and carers to ensure interventions are practical, sustainable, and person-centred.</li>
</ul>
<p>These actions are already making a tangible impact‚ supporting better outcomes, greater independence, and improved quality of life for the individuals we work with.</p>
