Posture Support in Occupational Therapy: A Holistic Approach

Occupational Therapists aim to support posture in ways that enhance comfort, function, and participation. Whether correcting or accommodating posture, the goal is always to improve quality of life across all environments, including beds, wheelchairs, armchairs, and vehicles.

Postural Alignment vs. Postural Perfection

Good alignment is desirable, but postural perfection isn’t always realistic. Each client’s tolerance, physical capacity, and lived experience must be considered. A comprehensive postural assessment—conducted in various positions such as lying, sitting, and floor-based—is essential to determine whether posture can be corrected or must be accommodated.

Tailored Equipment and Gradual Implementation

Once needs are identified, selecting the right postural support equipment is critical. With many similar products on the market, trialing is essential to ensure the best fit. For clients with complex postural needs, a graded introduction may be necessary to support tolerance and acceptance.

Posture is not just physical—it’s sensory, psychological, and habitual. Long-standing postural patterns can be difficult to change and require a sensitive, client-led approach.

The Role of Education in Posture Management

Postural support systems are only effective if those supporting the client understand how and when to adjust them. OTs are not present 24/7, so educating carers, family members, and support staff is vital. Movement is natural and influenced by pain, fatigue, emotion, and environment. Awareness of posture-related risks, such as pressure injuries, respiratory compromise, and reduced independence, is essential for proactive care.

Collaborative, Interdisciplinary Practice

Multiple systems influence posture: musculoskeletal, neurological, sensory, behavioural, and more. No single clinician can address all these areas in isolation. A collaborative approach ensures that:

Client-centered goals are meaningful and achievable

Strategies are consistent across settings

All members of the care team reinforce recommendations.

Interdisciplinary collaboration strengthens outcomes and ensures posture is supported holistically.

Posture as a Foundation for Participation

Posture affects how the body functions, recovers, and engages with daily life, not just during activity, but across the full 24-hour cycle. Poor posture can limit physical ability, cognitive engagement, and social interaction. When posture supports comfort and alignment, then clients are more likely to participate meaningfully in life.
